You can add an additional comment to the image that you just added to your notes.
This can be done by simply:
1. Click on the opportunity that you want to make a note about
2. Click on the 'Note' section
3. Type in the comment manually below the image or dictate to your device
4. Dictate your comment and save it by clicking the Back-arrow in the top left corner
